IMAGINE
“IMproving Antibiotic prescribinG In NursIng homEs”
IMAGINE aims to increase the prevention of infections – mainly UTIs – and reduce antibiotic inappropriateness for these infections by implementing a multifaceted intervention targeting HCPs in LTCFs including in particular nursing homes. The project applies the Audit Project Odense method of audit and feed-back before and after this intervention. 10 LTCF will be included in eight participating countries, seven of which are low-income member states (Greece, Hungary, Lithuania, Poland, Slovakia, Slovenia, and Spain) and Denmark.

From the 6th to the 9th of June our Scientific Liaison Officer, Georg Ruppe, participated in the Consortium Meetings of the EU projects IMAGINE and HAPPY PATIENT that took place in Copenhagen / Denmark
HAPPY PATIENT seeks to reduce the impact of AMR by decreasing the inappropriate use of antimicrobials for the management of common community-acquired infections: (1) Respiratory tract infections, (2) Dental infections, (3) Urinary tract infections This will be implemented in 5 Target Countries (ES, FR, LT, PL and EL) with diverse health systems, incomes and level of AM consumption.

AFFIRMO
"Atrial fibrillation integrated approach in frail, multimorbid and polymedicated older people"
The prevalence of atrial fibrillation (AF) amongst Europe's elderly population is growing. Associated with more severe strokes, AF is an abnormal heart rhythm with rapid and irregular beating. In the context of multimorbidity, improving the management of AF is vital and requires a holistic approach. In this context, the EU-funded AFFIRMO project will develop, implement and test the effectiveness of a patient-centred approach on older multimorbid AF patients in clinical practice. Specifically, the project will identify different clusters of multimorbidity in older patients with AF. It will also assess the needs of patients, caregivers and health professionals for the comprehensive management of multimorbidity including AF.

I-CARE4OLD
"Individualized CARE for Older Persons with Complex Chronic Conditions in home care and nursing homes"
Older patients with complex chronic conditions are mainly in home care and nursing home settings. Many times, healthcare professionals in these settings lack appropriate decision support. In this context, the EU-funded I-CARE4OLD project will develop and test next-generation decision support using high-quality internationally standardised routine care data based on a comprehensive assessment e-platform tool, InterRAI. Specifically, the project's multidisciplinary international consortium has collated longitudinal data from 52 million older recipients of home care and nursing home care from 8 countries including reliable, valid and harmonised comprehensive assessments of functional capacities, diseases and treatments. The focus was on predicting outcomes and the impact of treatments.
Objective: I-CARE4OLD aims to optimize healthcare for older patients with Complex Chronic Conditions (CCC) by improving individualized care. I-CARE4OLD will develop decision support for health care professionals for better prognostication of health trajectories and treatment impact based on a state-of-the-art digital platform using real world high quality data (particularly from InerRAI).

PRevention Of Malnutrition In Senior Subjects in the EU
With the European population growing older, the challenge is to keep an increasing number of seniors across all European countries healthy and active. In Europe, between 13.5 % and 29.7 % of older adults living at home are malnourished or at risk of protein energy malnutrition.
PROMISS (PRevention Of Malnutrition In Senior Subjects in the EU) aims to better understand and ultimately prevent protein energy malnutrition in seniors. Thereby, PROMISS will contribute to improve active and healthy ageing and is a multi-country project aiming to turn the challenge of tackling malnutrition in community-dwelling older persons into an opportunity for healthy ageing for the future. The PROMISS consortium contains worldwide expertise in epidemiology, clinical trials, geriatrics, nutrition, physical activity, microbiomics, as well as in behaviour, consumer, sensory and computer sciences. The multi-disciplinary international consortium will pursue the following objectives, divided in two phases: the understanding of the context and, based on this, the development of solutions to prevent malnutrition.

General Objectives
The project aims to use setting-specific Multidimensional Prognostic Indices (MPI) that predict mortality, based on a standard CGA, to improve the cost-effectiveness of health interventions in older individuals with multimorbidity and polypharmacy.
To do so, a review analysis on the use of predictive rules in clinical and management decision making in older residents in different European regions will be performed to identify reference models to allocate resources and lower costs in healthcare. Analyses of multi-national databases including information on drug prescriptions and older subjects characteristics will identify the best setting-specific MPI-profile in which individual interventions are more cost-effective in terms of improved survival.
The general objective is to reduce unnecessary use of health resources according to an appropriate and integrated multi-professional, planned interventions with frail and pre-frail older individuals with multimorbidity and polypharmacy.
